Admission Process- New Resident

 

At Golden Horizons, we deeply understand the significance of transitioning into an Assisted Living community, which is why we have meticulously crafted an eight-step admission process that places individual needs at its core. Recognizing that transitioning into a new facility can be an emotionally charged experience, we’ve designed our admission procedure with a singular goal in mind: ease and seamlessness. Understanding the inherent complexities and potential stressors associated with such a pivotal life change, our approach prioritizes clarity, transparency, and minimalism. By distilling each step into its most straightforward form and eliminating unnecessary hurdles, we aspire to alleviate apprehensions and streamline the journey to give you confidence and peace of mind. 

  1. From the moment you step into our premises, you’re met with warm, welcoming faces, setting the tone for the comprehensive experience ahead. Our talented team gives you a detailed folder that includes information about our community and contact information.
  2. Our dedicated team orchestrates a thorough tour, ensuring transparency by shedding light on every facet of our facility. We show you our apartment available to ensure clarity on expectations of living.
  3. Proceeding the introductory phase, our Director engages directly with you or your loved one in their office, facilitating an open dialogue to gather specific information about you or your loved on and address any queries, fostering trust and understanding. The Director will go through an inquiry form to get as much information on you or your loved ones capabilities and preferences.
  4. Once you have made a decision to chose us as your next golden home, our clinical team schedules a pre-assessment with you or your loved one, ensuring a relaxed environment as they delve deeper into understanding your loved one’s capabilities and needs. This pre-assessment can be done at our facility or at your home.
  5. Concurrently, we streamline the medical documentation process by liaising with your loved one’s medical providers, gathering essential details to tailor our care approach. Such documents are: medications list, diagnosis, clinical notes and any pertinent information about your medical history we should have on file.
  6. As the pieces fall into place and we have approved you or your loved one (meaning, your specific care is something we are licensed for), we coordinate a seamless move-in date, guiding you through essential admission paperwork day of move in. This paperwork portion usually takes 1-3 hours depending on conversation and questions that come up. You will need to bring forms of identification, security deposit, and insurance cards for us to keep on file.
  7. With an emphasis on individualized care, our clinical team conducts a move-in assessment on you or your loved one, paving the way for a personalized care plan that resonates with your unique requirements. This care plan is what our team of care providers will have access to so they can meet your individual needs and services when you join our Golden Horizons family.
  8. Once we have completed all the neccesary documentation, assessments and introductions, you can begin moving in, personalizing your new home and join the Golden family.
