Philosophy: To provide the frail and/or handicapped person the autonomy and support necessary to attain and maintain the psychosocial well being.
The Golden Horizons Assisted Living Community is semi-independent living in a private efficiency apartment. Each apartment has a private bathroom with shower plus ample living space which includes an area to put a microwave and small apartment-sized refridgerator.
Basic Services Include:
• Three (3) nutritional home cooked meals, plus snacks
• Once a week laundry & linen service
• Once a week housekeeping service
• 24-hour awake health care attendant for safety and security
• Arial Wireless Communication System
• Planned activities and outings
• Mail pick up and delivery
• Utilities (electricity, water and heat)
• Assistance in setting up medical and social appointments
• Assistance in arranging for or providing medical appointment transportation (some restrictions may apply)

• Licensed Nurse monitoring/service—charged for any person receiving a health related service (1 point)

• Basic Medication administration by attendant up to 4 times/day (2 points)
• Complex Medication administration by attendance more than 4 times/day (3 points)
• Verbal reminders (1 point)
• Supervision/reminders to get dressed or choose clothing (1 point)
• Daily physical assistance with dressing (2 points)
• Daily assistance with grooming (2 points)
• Assist with bathing/whirlpool once per week (1 point)

• Designed to provide temporary accommodations for persons who might be recovering after a hospital stay or for an individual otherwise being cared for at home whose caregiver needs a break.
Affordability
There are programs available for individuals with limited income through the County Human Services Agency to assist with the cost of housing and services. Please ask us for further details of where to go for assistance if interested/needed.
Description of Services
Standard Rent Package - The unfurnished apartment and basic care package.
Breakfast - Served at 8:00 am that includes toast, muffins, eggs, and hot or cold cereal.
Noon Meal - Typically the largest meal of the day. Served at 12 noon in our spacious dining room. Menu varies from roast beef and mashed potatoes and gravy to home made spaghetti and seafood.
Evening Meal - A lighter meal served at 5:00pm. Menu varies from homemade soups and sandwiches to hot dishes as well as seasonal favorites.
Light housekeeping - Light housekeeping includes dusting, vacuuming and cleaning the bathroom and floors. Does not include washing dishes, refrigerators, microwaves, etc. Also includes weekly bed linen change and laundered towels.
Laundry Services - Includes laundry pick-up and return to your apartment folded and/or on hangers. Does not include ironing.
Bathing/Shower Assistance - Assistance with shower or bath for safety/security.
Medication Reminders - Staff will come to your apartment and verbally remind you to take your medication. They will not order, refill or set-up medication.
Medication Management - A registered nurse will provide oversight and supervision for all medication management services. This may include oral as well as topical and inhaled medication as well as eye and eardrops. The RN will monitor your medication regime, accept orders from physicians, order and set-up all medications in coordination with the Physician and Pharmacist as well as observe for adverse reactions. In addition, the RN supervises unlicensed personnel in the provision of these services.
