Philosophy: To provide the frail and/or handicapped person the autonomy and support necessary to attain and maintain the psychosocial well being.
The Golden Horizons Assisted Living Community is semi-independent living in a private efficiency apartment. Each apartment has a private bathroom with shower plus ample living space which includes an area to put a microwave and small apartment-sized refridgerator.
Basic Services Include:
-3 Meals per day
-Laundry 1-2 x week
-Apartment Cleaning once a week
-Care Plan set up by Registered Nurse
-Monthly blood pressure checks and weight checks
-Planned activities and outings
-Mail pick up and delivery if desired
-Utilities
-Scheduled transportation for in town medical appointments and special activity outings

-Health Care Monitoring, room checks throughout 24 hour period to check on residents well being (1)

-Assisted Living Home Care RN monitoring (1)
